One time, I was doing Parkour over a rail, about up to my BB. I vaulted it, and my foot snagged. I fell on my face halfway through yelling "Crunch Time!". Had a 6CM bump on my head and a big nail scar on my eyelids. ALMOST Blind.
Also, once I was caught with a bunch of floppy disk bombs in my bag, I was gonna blow up the school computers to disrupt the school's economy forcing it to shut down till next month for another budget allocation.
AND I FORGOT TO SEVER THE TELECOMMUNICATIONS LINES!
Ugh... Stupid mistake.

I spent the weekend over at my mother's house, and she had an airbed for me to sleep on. The thing inflated itself pretty high off the ground, but... it was still pretty close to the ground, and me in my "just woke up" stupor didn't realize this. So, when my foot encountered the floor a little sooner than expected, I ended up tumbling right out of the thing and into her coffee table.
I need, like, a klutz prevention kit or something...

I chipped one of my false teeth - while on IRC no less - with those children toys like magnetix.
I like to chew things and stuff and if I have a pen It's right there. I didn't have a pen, I had a pen cap like shaped thing that resulted in a sudden motion and snapping together with another magnet, between my teeth which sent a chip.
A moment latter while seeing what happened I pushed to hard and caused that crack to extend and now a chunk is gone

When I was hoovering once, one of the headphones of my iPod shuffle was sucked into the hoover head. Being the curious fellow I am I lowered the iPod into the actual hoover (bearing in mind that it was still on...) and only realised how stupid it was when it was a long long way in. I started verrry slowly to lift it out, then the music I was listening to stopped. My headphones felt quite a bit lighter... my iPod was lost in the hoover! So I had to quickly open it and break apart the entire hoover bag (full of dust/ dirt/ wrappers) to find it again. Bleh. Funny day =]
When I was little,there was a time when my family was at a relatives house preparing fresh fish to cook. Before my parents gutted the fish and everything, I decided to look at them. I was about there when a little girl (a cousin, probably) flipped the plate accidentally. Before I knew it, I was on the concrete with a concussion from several fish hitting my face. It's an epic fail on my part, really.

Speaking of concussions, I gave myself a concussion when I was really little. Still have a dent in my skull from it.

When I was 15 and learning to drive my mother and I went to the store and while she was in the store I went over to the movie rental place to drop off our rentals and I scratched the crap out of the side of my mom's car on the drop off box but didn't tell her cause I wasn't supposed to be driving. Then a few weeks later she went to rent a movie and saw the white paint (her car was white) all over the drop off box :S
One day at school, during winter, I put my foot up on top of a wall glazed with ice t tie my shoe lace. Next thing I know I'm waking up with lots of people staring down at me with my glasses bent around my face. People say I went to hop over the wall after I tied my shoe, but I swear I fainted, cause I briefly remember a dizzy feeling before I fell. Or at least I think I can, don't remember too much.
Oh and in the same month I was pushed down a slippy hill in school at got my feet caught under the fencing. I was pelted with ice (the snow had frozen) for about half an hour before I got my feet out. Wasn't very nice, infact it was fucking freezing, and it only enflamed my hatred towards many people
