In risk assessments the floor basically opens up for suggestions from everyone, I have a suspicion that "mini black holes could be created and then maybe they can combine and grow and suck up the entire Earth" was one of these - unfortunately newspapers caught wind and ran with it. Just a theory.
For me the common risk assessment is "a bus could smash through our wall and destroy everything/kill everyone", I expect someone who isn't clued into risk assessments (particularly sarcastic British humour) would actually take this one seriously, when in reality it exists as a pseudo guarantee that we covered everything and had to open up the floor to unlikely scenarios.
I understand that teachers in UK schools tend to do this for risk assessments when doing school-trips. So if going on a school trip to the zoo, one of the risk assessment items could be "the lions could escape and eat literally
all the children" and that would be at the end of the list of more serious, statistically realistic concepts.
