Looking for an old Game Over Script

I used to have this script that replaced the game over script.

It's exactly like the Advanced Title screen script where you use images. The script would give you a menu, but in the form of images like the Title screen.

Does anyone have this? I can find it on this forum or the Creative Asylum forum [where I got it originally].
