Easy Request ;) Add State for Low HP

I already created a state called "Injured"
Basically, I want it to add the state injured whenever the hero's hp is at 25% or less. I have no clue how to add a state in script ':|

lol, im pathetic... please help. I know it can be done in common event, but I wanna learn script, and learning code structure would be valuable. Thanks so much.
