detailed CMS request.... please help

ok im editing my post here is what i was looking for. i would like a custom menu that looks like this

but i would like it to have the features of this one, only not the beastery

ok as far as i can tell this should be about it and if you need any more info please tell me ill be hoping that someone can do this my game relly needs this (and of course your name will be in my credits, this game when im throu im hopeing to sell.)

Please try to explain it better and if you could, provide pictures and sounds you want to use in your menu. How should the options look like, etc.
