Designing a program

Greets everyone,
I've started to work again on designing a program for someone else to code. Problem is I'm not exactly sure what's required as in individual bits and bobs.

I've got a concept of the final visual structure and some .png images to be used within a toolbar. Got a few questions but if there's anything that yous know I've overlooked could you please let me know as before I ask for this to be created I'd like to know all the other work is ready.

1. How's the best way for me to create an icon? Since I've only ever previously made ones under 98 where I needed a tiny and a small icon.

2. Within the program and with it being directed to a folder containing images, would it be possible to have similar file names linked/grouped where only the last two or three numbers being similar. -This'll be using a drop down scroll selection box.
Then with that a second drop down selection box underneath that contains the individual files of that group?

Basically as an example linking to a folder called Watch, then within that Watch folder there is DVD***.png VHS***.png BluRay***.png
DVDs go up to 075 (75 files like DVD001.png DVD002.png etc) VHS up to 430 and so forth.
-From the first box you select one of the unique names from that folder, then the second box updates with *** how many images there is from that list.

3. How easy would it be to create a .gal image file which would contain the layer information?

4. When selecting a image from the list could it be possible to off centre the image depending on which selection button is set?
Example would be say there's two dimensions of boxes for the images I'm selecting (say normal one at 200 x 200 and the other 150 x 200), one is fine and can be set to start from 0,0 where if a button has been pressed moves the image to -50,0 meaning it doesn't show the first 50 pixels.
