Customized "Choice" Script

hello XDD
I wanted a script more or less well ...
My game is going to have some artifacts 5 in truth.
When push C  appear some options with the icon of artifacts and each artifact has a specialty.
Names:
-Muscle-Gem (increases power)
-Valkyrie Helm(Helm of a valkyrie)
-Finish Glove (raises things)
-Final Orb(power forbidden)
-Syrius Arm (transforms the arm in arm of the character of Syrius, with great ability, and that when equip change the picture (char) of the character.

I want that,when i equip the valk helm..i want a helm in the head of my char .

I wanna a window with the name of the artifacts and a descrition...
