F.Y.I. - I used to provide free web hosting to people. We had over 2,000 accounts set up within about a half a year...and it didn't cost anything to setup hosting. An administrator goes into the WHM (web hosting manager), clicks on 'create a new account', types in the desired domain, user name, password...and then it generates the account. All the company needs to do after this is point the domain to the appropriate DNS...which take a whole five seconds.
Setup fees are a rip off and any 'company' that forces their customers to pay a setup fee...should be drawn and quartered. Go with a hosting company that doesn't charge a setup fee...I guarantee you will be happier and save a bit of money at the same time.
