Where do you get the Ideas, and how do you make those Ideas real?!

For the longest time, I've seen many great programs, and applications. But when I try to make one of my application ideas come to life, I found myself wondering how to make it happen. I have a question for all of you REAL programmers.

                How do you come up with your ideas for you programs or applications?

                And also, how do you make those Ideas come to life? How do you know what to do?
