Any Number Bandwidth (As long as it is good enough..:P)
Allowed to use FTP
PHP
A nice amount of Space (Memory, like, a good amount....1 GB?)
FREE TOP LEVEL DOMAIN NAME (.com, .net, .org, etc)
NO SETUP FEES

They are all good hosting companies.  I've used them all before.  And if you're looking for a cheap site to host both your website and your files, DON'T.  This will use up bandwidth really, really fast (unless your games suck).  Use this file hosting sites for that (free or paid):

I've used both 1and1 and Dreamhost for shared hosting.

For your specs Dreamhost would give you:
500GB hard drive
Free Domain
PHP 4 and 5 support
MySQL 5 Database: unlimited number and size limited to hard drive space.
24 hour email support only.  No phone.
$5.95

In that scenario you would be on a shared host.  My only critique is uptime has been a problem for Dreamhost of late, but I don't think its any more than any other shared hosting provider.  Their support is very helpful and often gets back to you quite promptly.  Here's the plan details.  If perfect uptime isn't as big of a deal to you as memory space and flexibility of access and use Dreamhost may be what your looking for.


1and1 has been quite stable for me.  Here's what they would give you on their shared hosting plan:
120 GB hard drive
Free Domain
PHP 4 and 5 support
MySQL database: Maximum size 100MB's and you are limited to 25 databases (non negotiable, I've asked)
24 hours phone and email support.

If uptime is important 1and1 seems to be the more stable host in my experience.  However, if you wish to host a forum here you will need to prune it semi-regularly to keep the database within the constraints.  Also talking to a human instead of sending emails is nice as well.
