Need to clarify on states/elements efficiency.

I never fully understood states/elements efficiencies [what the letters mean] so I was looking around the boards and I found this much [I also checked in the help file for VX, couldn't find anything]:

A - Large Damage Increase (Double damage, I think)
B - Small Damage Increase (Around +25%, I think)
C - Normal Effect
D - Damage is Halved
E - Nullify (No damage/Effect)
F - Reverse Effect (Will heal instead)

Now my question is probably an obvious one, but does this refer to the actor that the efficiency is assigned to, or the enemy? Same for enemies, does it mean for example an enemy with an A state efficiency will double the damage that their attack gives, or does it basically mean that any attack received with that state/element will be doubled against them?
