Custom Mog Styled Menu Help

Ok I installed the Mog styled Menu script:

Here is what I would like my menu to look like:

Could anyone help me on this? I am looking thru the script and tring to get it to look this way. I know that I have some features that isn't part of the script, which is ok, would still like the place holder there for now.

are you asking for the picture or the script?
Im no good at scripting but i'm good at making pictures, so if you would like, i can help you out with that.

do you want all of those options (order, formation, config, and party)? and do you want it to have the fifth member slot?

also, do you want the graphics to look plain like that or do you want it to look all flashy and stuff, what colors lol, i need all the details on it.

I got the script installed and the pictures the way I want them to look, I just need to get the script and the menu to match. So looking for help on the script.
